Students Taking Medication at School
Please see the attached letter in regards to students who need to take medication at school. Also included is the medication form required to be filled out for any student needing to take medication at school.
Students with Asthma
Attached is the asthma action plan form students. Forms need to be completed by a physcian.

Food Allergies/Substitutions:
Parents/Guardians, if your child has any food allergies, substitutions, or other requirements, please take a mom to fill out the attached form. The district has a new food provider.
Parents of 5th grade students:
Before entering 6th grade, students will need a physical and dental exam, as well as immunization requirements. Please use the following links for the required forms. Thank you!
* Physical Exam must be on the required Illinois form (Sports physicals are NOT acceptable.) Parents, please be sure to complete the yes/no

* Complete immunization record included (1) Tdap, (1) Meningitis, (3) Hep B
* Dental exam must be on required Illinois form and is due May 15th.
